To get started, visit our home page at www.dboone.org and click on the Student Registration tab from your computer. Documents needed: Child's birth certificate (Passport, baptismal certificate, or notarized statement indicating date of birth also accepted) – REQUIRED at time of registration. A copy of the official immunization record signed by your doctor with the month, day, & year of each shot - REQUIRED at time of registration Your child will need a physical examination and dental check-up. The physical exam form needs to be completed at the five-year check-up. Proof of residency: Copy of deed, sales or lease agreement, utility bill, property tax bill, etc. - REQUIRED at time of registration If applicable, provide custody papers, special education documents (IEP, ER, RR, NORA), and legal documents.
