2025-26 Kindergarten Registration
Kindergarten Registration will open in March of 2025!
Daniel Boone Area School District offers a full-day kindergarten program. PLEASE NOTE: Registration will not open until March 2025 for the 2025-2026 school year. Children must be five years old before September 1, 2025 to be age eligible for kindergarten. Transition events for you and your child, including tours, etc. will be completed at a later date.
Parents must provide the following documents for registration to be completed:
Child's birth certificate – REQUIRED at registration (Passport, baptismal certificate, or notarized statement indicating date of birth are also accepted)
A copy of the official immunization record signed by your doctor with the month, day, & year of each shot – Your child’s doctor will be able to give you a copy of this record - REQUIRED at registration.
PA State Law requires proof of these immunizations:
4 doses of Tetanus and Diphtheria (DTP, DtaP, DT, or Td-one dose after 4th birthday)
4 doses of Polio (OPV, E-IPV)4th dose on or after the 4th birthday and at least 6 months after the previous dose given
2 doses of Measles, Mumps, and Rubella (MMR-1st dose given on/after 1st birthday)
3 doses of Hepatitis B (Hep B)
2 doses of Varicella vaccine
Completed registration forms (located on our Student Registration page).
Your child will need a physical examination and dental check-up. The physical exam form needs to be completed at the five-year check-up. Please note: Parents should have the physical form completed by the doctor's office with the immunization records and the latest dental visit form. Parents must upload that document to the portal or contact the school nurse with the date of the physical or dental visit scheduled before the start of the 2025-26 school year.
Proof of residency. Examples include:
Copy of deed, sales or lease agreement, current utility bill, current credit card bill, property tax bill, vehicle registration, driver's license, Department of Transportation identification card or a piece of mail sent by the federal or state government such as benefits information, voter registration or tax documents will be accepted.
If the student and/or family are living with friends or relatives, a notarized statement listing the occupants of the household (including the student/family), signed and dated by the owner of the property is required.
Original Passport/Visa if not a citizen
If the child does not live with both biological parents, please provide custody papers indicating “Custodial Parent” or “Guardianship”.
Any other legal documents
Special Education, such as IEP, ER, RR, NORA (if applicable)
